What information and documents you need for the MDAC

A complete checklist of the information and documents required to submit a Malaysia Digital Arrival Card without errors.

The MDAC is a short form, but every field has to match supporting documents exactly. Travellers who prepare the right information in advance can usually complete the declaration in under five minutes. This article lists everything that should be on hand before starting the form.

Passport details

The passport is the central document for the MDAC. Have it open at the photograph page when filling in the form, and confirm the following:

  • Full name as printed in the passport, including all given names
  • Passport number exactly as shown
  • Nationality as indicated on the passport, not the country of residence
  • Date of birth in the day-month-year format requested by the form
  • Date of issue and date of expiry
  • Country of issue

The passport must be valid for at least six months beyond the planned date of arrival in Malaysia. A passport that meets this rule on the day the MDAC is submitted may still be rejected at the border if it falls inside the six-month window by the date of travel, so always check the expiry date carefully.

Travel itinerary

The form requires the date and mode of arrival, the carrier and the point of entry. Travellers should prepare:

  • Flight number, airline and arrival date for air travellers
  • Vessel name and voyage number for cruise and sea arrivals
  • Vehicle type and border post for land arrivals
  • Connecting flight details only if Malaysia is the final destination

If a return or onward ticket is required by the airline at check-in, keep that booking accessible. While the MDAC itself does not require proof of onward travel, immigration officers may ask to see it on arrival.

Accommodation address

An address in Malaysia is required for the first night of the stay. Acceptable formats include:

  • Full hotel or serviced apartment name, with city and state
  • Street address of a private residence when staying with friends or family
  • Name and city of a homestay or guesthouse booked through a recognised platform

Travellers on multi-stop itineraries within Malaysia only need to declare the first address. There is no requirement to list every subsequent hotel or destination on the MDAC.

Contact information

A working email address is mandatory because the confirmation is sent there. Travellers are advised to use a personal email rather than a workplace address that may filter unknown senders. A mobile number is requested but is not always mandatory; if provided, it should include the international dialling code.

Purpose of visit and length of stay

The form asks for the main purpose of the visit, with options that typically include:

  • Holiday or tourism
  • Business meetings or short assignments
  • Visiting friends or relatives
  • Education or training
  • Transit
  • Medical treatment

Select the option that best describes the primary reason for travelling. The length of stay must be stated in days and should match the duration permitted by any visa or visa exemption that applies to the traveller's nationality.

Documents you do not need for the MDAC

The MDAC itself does not require uploads, scans or photographs. There is no need to provide:

  • A copy of the passport biographical page
  • Hotel booking confirmations
  • Bank statements or proof of funds
  • Health insurance certificates
  • Visa documentation, beyond entering the visa number where requested

Some of these documents may still be requested by the immigration officer at the border, but they are not part of the MDAC submission itself. This is one of the reasons the form is straightforward to complete from a phone or tablet.

Special cases: children, group travel and dual nationals

Each traveller, including children and infants, requires an individual MDAC submission. A parent or accompanying adult can complete the form on a minor's behalf, but the personal details must be those of the child.

Dual nationals must use a single passport consistently for the MDAC, the airline booking and the immigration entry. Mixing passports between any of these steps is a common cause of mismatched records and delays.

Group travel arrangements, such as organised tours, do not exempt individuals from the MDAC requirement. Tour operators sometimes assist with bulk submissions, but every individual is ultimately responsible for the accuracy of their own declaration.

Final preparation tip

Many delays at the immigration counter are caused not by missing documents but by small typing errors in the MDAC. Reading every field aloud against the passport before pressing submit is a simple discipline that catches most mistakes.
